Currently, there is a rapid growth of domestic manufacturers of medicines, which determines the need for a highly qualified R&D team with specialized competencies in the field of pharmaceutical development. The analysis of regulatory documents regulating the field of industrial pharmacy, master’s degree educational programs implemented in the territory of the Russian Federation, and the opinions of current employees of an organization engaged in pharmaceutical development revealed the need to develop and justify an approach to the training and development of specialists in this field. OBJECTIVE: to substantiate an approach to the education and development of specialists for pharmaceutical development, based on best practices and specialized knowledge and skills of employees of R&D departments. MATERIALS AND METHODS: regulatory documents and professional standards in the field of industrial pharmacy, recommendations for the design of educational programs, the results of a survey of employees of the company developing reproduced medicines were studied using the sociological method (questionnaire), structural–logical, comparative and correlation analysis. RESULTS AND DISCUSSIONS: during the study, aspects of training and development of employees of R&D departments were analyzed on the basis of scientific papers, professional standards of specialists in industrial pharmacy, methodological recommendations for the development of educational programs and the experience of the directorate of the organization developing reproduced medicines. Based on this, a survey was formed, which includes 111 questions formed into 4 independent blocks: determining the specifics of staff training, the relevance of additional education for pharmaceutical development specialists, the importance of general knowledge and the importance of specific knowledge depending on the department of work of employees. Individual questions were evaluated using the Likert scale, placing the significance of the answers in accordance with the scores. According to the results of the survey, the areas of general and specific knowledge and skills were identified and ranked. Based on professional standards and survey results, the necessary specialized competencies of R&D specialists are substantiated and formulated. The conducted study allowed us to propose an approach to the formation of an educational program for additional professional education of specialists in the field of pharmaceutical development. Conclusion: the implementation of the proposed approach to the formation of an educational program of additional training with its subsequent implementation at the master’s level is expected to provide training for specialists in the field of pharmaceutical development who will be able to effectively solve the tasks of developing effective, safe and high-quality medicines, implementing a drug safety strategy in the territory of the Russian Federation.

The Ministry of Health of the Republic of Tatarstan, with the methodological support of the Department of Organization of Drug Supply and Pharmacoeconomics of the Institute of Postgraduate Education of Sechenov University, conducted a comprehensive analysis of the rationality of the generated application for 2024 and an analysis of the effectiveness of spending money on the purchase of drugs at the level of medical organizations, followed by the development of an algorithm for generating an application for the purchase of drugs. The volume of the application in monetary terms exceeded 7 billion rubles. The analysis, with data on the initial and adjusted applications, showed that the use of a number of drugs not from the list of vital and essential drugs in the MO was due more to established algorithms and “work habits” of individual doctors. In this regard, a corresponding adjustment was made. As a result, based on the results of the work, corrections were made to 360 INNs for a total of 122.5 million rubles. The result of the work carried out was the development and approval of the Order of the Ministry of Health of the Republic of Tatarstan No. 2436 dated October 17, 2023 “On the optimization of the formation of applications of healthcare institutions”, according to which a working group was formed and a new application form was approved.

INTRODUCTION. Gastroesophageal reflux disease (GERD) is an actual problem of practical healthcare, which is also caused by high financial costs. The latter need a reasoned correction. THE AIM is a pharmacoeconomic analysis of GERD maintenance therapy based on clinical recommendations. MATERIALS AND METHODS. 104 GERD patients were selected from 2,130 respondents. They performed ABC/VEN and frequency analysis of drug therapy: alginates; proton pump inhibitors (PPIs), antacids; H2-histamine receptor blockers (BN2GR), prokinetics, esophagoprotectors (EFP), ursodeoxycholic acid preparations (UDCA) and other medicines (adsorbents, antibiotics, as- tringents, choleretics, probiotics, antispasmodics and enzymes). A cross-sectional simultaneous study was conducted. RESULTS. 104 patients spent 6476.5 rubles/day on GERD treatment at home: group A – 5184.8 rubles/day; group B – 975.4 rubles/day and group C – 316.3 rubles/day; category V – 51.1% of the cost of all drugs, category E – 27.2% and category N - 21.7%. In class A, category V – 62.1 rubles/day (2.7% of the total cost of Class A drugs), in category E – 3033.1 rubles/day (58.1%) and in category N – 2089.6 rubles/ day. (39.2%). Frequency analysis of drugs from categories V and E showed that the overspent amount of drugs could have been purchased: in the PPIs group – 24 doses of Omeprazole; in the prokinetics group – 13 doses of Itopride; in the antacid group – 20 doses of Almagel®; in the EFP group – 2.4 doses of Alfazox. The cost of the 3 drugs used from the UDCA group was the same. Alginates and BN2GR were accepted by the same name. The same situation was found in category N, with overexpenditure ranging from 1 to 29 doses in the compared groups of secondary drugs. CONCLUSION. The relevance of GERD is due not only to its fairly widespread occurrence, but also to the high financial costs, which do not always take into account financial expediency. Its increase will improve the effectiveness of GERD maintenance therapy.
